From group workshops to one-on-one support to educational materials, Kern Family Health Care (KFHC) makes it easy for you to get the help and support you need.

Asthma Management

KFHC offers the following asthma education for members interested in managing their asthma better.

Asthma Classes

Members will learn about how to manage their asthma and breathe better.

Class topics include how to:

  • Get asthma under control
  • Manage common asthma triggers
  • Clean your home with asthma friendly recipes

What are Asthma Preventive Services?

Asthma Preventive Services (APS) include clinic-based asthma education, home-based asthma education, and in-home asthma trigger assessments. Community health workers who are trained to offer asthma education and in-home trigger assessments provide APS to members who have asthma.

Who is eligible for APS?

KFHC members must have been diagnosed with asthma before they can receive APS. They will also need a recommendation for APS by a licensed health care provider. Members must have poorly controlled asthma or the recommendation of a licensed health care provider to be able to receive an in-home trigger assessment. Poorly controlled asthma is defined as having one of the following:

  1. An Asthma Control Test score of 19 or lower, or
  2. An asthma-related emergency department visit or hospitalization, or at least two instances of asthma-related sick or urgent care visits in the past 12 months.

An assigned APS provider will assist each requesting member with reaching out to a licensed health care provider for a recommendation for APS, if needed. The APS provider can also help each member access asthma care.

How can members request APS? How can they be referred?

Members can request APS by calling KFHC at (661) 632-1590 or 1 (800) 391-2000. When speaking with Member Services, they can request APS. KFHC staff will call members back to find out if they are eligible for APS. Then they can refer members to an APS provider, if needed.

Diabetes Prevention and Management

Diabetes Prevention Program

Kern Family Health Care (KFHC) is offering the Diabetes Prevention Program (DPP) at no cost to members at risk for diabetes. This is a lifestyle change program made to prevent or delay type 2 diabetes. They where members will:

  • Have one-on-one support with a lifestyle coach.
  • Learn how to meet set goals and new skills.
  • Have access to a support group of people with similar goals and challenges. Members can share ideas, celebrate successes, and work together to overcome challenges.
  • Track their goals, including routine weigh-ins at each class.
  • Get rewarded for attending and reaching goals.

Our coaches make the program fun and engaging with cooking demonstrations and interactive activities.

How long is the program?

  • The program lasts for 1 year.
  • Meetings are held once a week for the first 4 months and once every 2 weeks for 2 months.
  • During the last 6 months, meetings are once per month.

Diabetes Empowerment Education Program

KFHC offers the Diabetes Empowerment Education Program (DEEP) to members living with diabetes. The program is a weekly 6-class series.

DEEP covers 8 topics:

  • Know the human body and risk factors for and with diabetes
  • How to check your body, use medication, and get medical care
  • The importance of physical activity and meal planning
  • Getting support from family and friends

Who can join?

  • 18 years or older
  • Diagnosed with diabetes or at-risk of developing diabetes
  • Caregiver to a person with diabetes

Heart Health Education

Our heart health education classes are a vital part of preventive care, helping members take charge of their well-being through learning and adopting lifestyle changes.

Heart disease is one of the leading causes of death in the U.S., but many cardiovascular conditions can be prevented. Our classes are designed to help you understand your risks and take proactive steps toward a healthier heart.

Class Topics Will Include:

  • How blood pressure and cholesterol levels affect your risk for heart disease and stroke.
  • Nutrition education, highlighting the benefits of the DASH Diet.
  • Work toward a healthy weight
  • How to use a blood pressure monitor
