The Baby Steps Plus Program is here to provide information and help decrease and prevent complications to help you have a healthy pregnancy. This program provides support for high-risk pregnant women, some of the support this program offers:
- Getting a prenatal care as soon as they find out they are pregnant and attend all recommended prenatal and post-partum appointments.
- Provide care management and care coordination.
- Obtain timely newborn check-up and immunization.
High Risk pregnant members can choose (voluntary) to enroll at any time during their pregnancy into this program. Some other qualifications are:
- A High-risk pregnancy is definition by Preexisting health conditions.
- Pregnancy-related health conditions
- Lifestyle factors (including smoking, drug addition, alcohol abuse and exposure to certain toxins)
- Age (being over 35 or under 17 when pregnant)
Services that Baby Step Plus Program offers:
- Your own health navigation/care coordination to help connect you to supportive programs such as high-risk obstetric diagnosis and treatment.
- Assist with connecting you to different programs such as WIC, CalFresh, Medi-Cal, and health insurance enrollment assistance for the entire family.
- Offer short-term and long-term care management.
- Home visits by a Registered Nurse until baby is 2 months old.
- Provide education regarding prenatal and postpartum care, breastfeeding issues, signs and symptoms of preterm birth, dangers of smoking and substance use during pregnancy, labor and delivery issues, and infant development.
- Screen all members for psychological issues and refer to Behavioral Health services.
The program is free for KFHC Members.
Once you know you are pregnant, you can be referred to the program by:
- Calling our Member Services department at 661.632.1590 (Bakersfield) or 800.391.2000 (outside of Bakersfield)
- A referred form can be submitted to us by your providers office, or a community partners.
