KFHC CCS Navigators will assist CCS eligible members with their transitioning planning when they are transitioning out of CCS services. This transition planning will help CCS member and their family prepare move from a pediatric to an adult coordinated model of health care.
- All KFHC members who have at least one eligible medical condition under California Children's Services or diagnosis between the ages of 18 to 20 years of age.
- Currently enrolled with KFHC.
- Between the ages of 18-20 years old.
- Has at least one CCS eligible condition and open to CCS.
- Transition planning services includes:
- Transition letter sent to members ages 18, 19, and 20 years by mail to remind members they are aging out of the CCS program, and offer the support needed.
- Letter will include contact information about access to Medi-Cal, primary care physicians and or specialty care as appropriate.
- KFHC staff will call member for follow-up until the member ages out of CCS
- KFHC staff will offer Case Management services, if needed.
There is no cost to KFHC members.
KFHC can assist with transportation to your doctor's appointments.
