Children with Special Healthcare Needs (CSHCN)

The Kern Family Health Care (KFHC), Children with Special Health Care Needs (CSHCN) program helps children under 21years of age who have complex health problems. This program has a special team that works together to help members who are in the CSHCN program and their families.

If you are receiving California Children’s Services (CCS), we will work with the CCS Case Manager to make sure the child’s health care needs are met.

Who can be part of the program:

  • KFHC active members
  • Under 21 years of age
  • Have at least one complex chronic condition

Program services:

  • Help the member to follow their care plan and make their lives better.
  • Ensure all the member’s doctors and care teams work together. This includes the member’s:
    • Primary Care Provider (PCP)
    • Specialty Care Provider
    • Other medical resources
  • Help guide the member to move from CCS to adult medical care before they turn 21 years old.
