Chronic Obstructive Pulmonary Disease (COPD) Program

Many people around the world suffer from Chronic Obstructive Pulmonary Disease (COPD) for years and die due to the complications. COPD is commonly recognized by chronic respiratory symptoms and airflow limitations caused by airway and/or alveolar abnormalities. The symptoms usually caused by significant exposures to environmentally unfriendly particles such as tobacco. Some other factors including anormal development of the lung. KFHC has partnered with a provider to ensure that our members with COPD receive comprehensive treatment and care, along with home visits by a respiratory therapist and/or Durable Medical Equipment (DME) Provider.

Who is Eligible?

KFHC members diagnosed with COPD needing comprehensive care management

Services Provided

  • Evaluating and providing treatment for members to decrease avoidable and unnecessary readmissions.
  • Ensuring members participate in pulmonary rehab care.
  • Working with providers and members to establish coordination of care back to primary care provider.
  • Encourage tobacco-using members to receive tobacco cessation counseling intervention.
  • Working with Respiratory Therapists and/or Durable Medical Equipment (DME) Provider to reduce the risk of COPD exacerbation.

Is there a cost for this program?

There is no cost for this program to KFHC members.

How to get referred?

KFHC will identifies high-risk members with COPD needs. Once members are identified they will be referred to the program.
