Complex Care Management (CCM)

Complex Care Management (CCM) is a team that connects with our members who have experienced a major health event or diagnosis that requires extensive care to the care and services they need. Assist our members in understanding the health care system to receive the best care possible. Our goal is to help you have the highest quality of life and to better explain your Primary Care Provider's (PCPs) plan. These efforts help to lower costs and improve quality through focusing on the delivery of care in the right time and place.

Who is Eligible?

KHS Members who experienced a major event or diagnosis that needs an extensive care coordination and need assistance navigating the medical care system to connect with different services and resources.

Servies

Complex Case Management includes Basic Care Management such as:

  • Process of assessing, planning, and assisting you in meeting your health needs.
  • Assist with educating and facilitating your visits for services provided by your doctor.
  • Support Primary Care Provider (PCP) as the primary source of your healthcare and source of specialty referrals.
  • Eliminate potential barriers to getting the right care for you within your medical home. This may include housing, transportation, childcare, nutrition, access to the right doctors, discharge and transitional care planning, and health education.

Is there a Cost for this program?

There is no cost to KFHC Members.
