Emergency Room (ER) Navigation

KFHC has a specialty program called the Emergency Room (ER) Navigation that is designed to identify KFHC members prior to a potential preventable admission (PPA). This program helps to minimize these types of admissions by promptly directing care to an appropriate setting all while providing treatment to address the acute issue. Treatment will be coordinated with care coordination, care navigation, and appropriate condition-based education.

Hospital admissions that could possibly be addressed in an outpatient setting are referred to as Potentially Preventable Admissions (PPAs). It is found that these types of hospital admissions were due to:
• Hospital and/or outpatient care ineffectiveness
• Lack of adequate access to outpatient care services
• Inadequate coordination of outpatient care services
• Inadequate monitoring and appropriate follow-up treatment for chronic conditions

Who is eligible?

All KFHC members admitted and/or seen in contracted hospitals as inpatient, or in their emergency rooms.

Services Provided

  • Directing care to appropriate medical treatment while attending to their current medical needs.
  • Provide care coordination
  • Care navigation and appropriate condition-based education.

Is there a cost for this program?

There is no cost for KFHC members.
