Long Term Care Program
Kern Family Health Care (KFHC) offers a Long Term Care Program that includes Complex Care Management (CCM) benefits to help members who are staying in, or are trying to get placed in a Long-Term Care (LTC) facility.
These services are given to you to get the help you need to better your health. The LTC Team works with you, your health care team including your doctors, caregivers.
KFHC's LTC Team works with your LTC facility to bring you:
- A team to review your health care plan
- Care team to work with your doctors
- Case managers
- Enhanced Care Management (ECM) and Community Supports Services (CSS) team referrals
- Inpatient care team
- Local social services
- Long term services and supports (LTSS)
- Nurses
- Outreach teams
- Reminders when a LTC center is ready for you
- Skilled Nursing Facility (SNF) care team
- Social workers
