The KFHC Palliative Care Program is a dedicated community/home-based care program for our members. Unlike hospice care, which provides a comfortable environment for those in the final stages of life, palliative care is appropriate for any stage of serious illness and any age. The Palliative Care Program consists of consultation visit. This will include and assessment for eligibility for the program enrollment when criteria is met as documented on the Certification of Advanced Disease (CAD).
All KFHC member that meet the Certification of Advance Disease (CAD) criteria and has high acuity risk factors, regardless of age.
Palliative Care must include a minimum of the following services regardless of age, and with a medically necessary reason for palliative care or management of a qualifying serious illness:
• Advanced Care Planning
• Palliative Care Assessment and Consultation
• Individualized Written Plan of Care
• Pain and Symptom Management
• Mental Health and Medical Social Services
• Care Coordination
• Palliative Care Team
• Telephonic Palliative Care Support
There is no cost for KFHC members.
