KFHC Transition of Care (TOC) program provides a broad range of time-limited services available for all KFHC members. This includes members transferring from one setting or level of care to another to ensure continuity of health care and avoid poor health outcomes. This will help with ensure a safe and timely transfer of patients from one level of care to another from one type of setting to another.
KFHC members who were admitted to acute hospitals are assessed for eligibility.
Some services provided are:
- Engaging with members while in the hospital
- Assessing members risk for readmission
- Securing discharge follow up appointment and addressing care coordination for member needs prior to discharge
- Working with providers and members to coordinate care back to primary care provider.
There is no cost for KFHC members.
KFHC can assist with transportation to your doctor's appointments.
