Population Health Management (PHM) Triage Team evaluates and connects KFHC members to PHM Programs that provides care coordination services. The Triage Team will ensure KFHC members receive timely assistance and improve health outcomes in a multidisciplinary approach. These services are based on the KFHC member needs.
The PHM Triage Team assists with making sure the proper referrals are sent to the PHM program. This will help ensure members meet criteria for programs and will help improves timelines for member outreach. In addition, the team provides education, resources, and helps redirect referrals as need to appropriate KHS programs.
There is no cost to KFHC members.
- Member's can request for Case Management via phone call.
- Provider can refer members to our PHM Triage Team.
- Request can be made thru our Member Services department.
- Provider can send referrals thru the Provider portal.
- HRA results for High-Risk SPD’s.
